Policy for the protection of personal information for patients receiving services

The Customer Privacy Policy is established as the basic policy of the Medical Corporation (“Notice 1”) with regards to the handling of personal information of persons receiving our services (hereinafter referred to as a “Customer”).
Specifically, the personal information referred to here is the personal information designated by Article 2, Paragraph 1 of the Act on the Protection of Personal Information.

Notice 1: our Medical Corporation (Midtown Clinic Medical Corporation) operates the following medical institutions. Tokyo Midtown Clinic, Tokyo Midtown Dermatology and Plastic Surgery Clinic (Noage), Tokyo Midtown Dental Clinic, Tokyo Midtown Center for Advanced Medical Science and Technology, Midtown Clinic Ariake, Midtown Clinic Tokyo Bay, Nihonbashi Muromachi Mitsui Tower Midtown Clinic, Yamanakako Clinic

Notice 2: Act on the Protection of Personal Information (May 30, 2003; Act No. 57) Article 2 “Personal information” in this Act means that information relating to a living individual who falls under any of each following item:

  • (i) those containing a name, date of birth, or other descriptions etc. , whereby a specific individual can be identified (including those which can be readily collated with other information and thereby identify a specific individual)
  • (ii) those containing an individual identification code;

Policy for the protection of Customer's personal information

01 Collection of personal information

In case of collecting the personal information from the Customer, the Medical Corporation shall acquire the information by proper means.

02 Utilization purposes of personal information

The following are purposes that the Medical Corporation shall utilize the Customer's personal information.

1. Utilization inside the Medical Corporation

  • For providing healthcare services
  • For medical billing related affairs: Clerical work regarding healthcare insurance, long-term care insurance, industrial accident insurance, and publicly funded healthcare.
  • For administrative operations. Creating of basic documentation for the maintenance and improvement of accounting, bookkeeping, internal reporting, healthcare services, etc.; cooperation for healthcare intern; clinical case study and system development with the purpose to contribute to healthcare improvement and dissemination of information to the public.

2. Utilization in the contracted services

  • Contracted laboratory services, etc.
  • Contracted services regarding healthcare insurance, long-term care insurance, industrial accident insurance, and publicly funded healthcare.
  • Contracted services for information maintenance and management
  • Research and system development with the purpose of improving healthcare and dissemination of information to the public

3. Utilization for provision of information

  • Coordination with other hospitals, clinics, pharmacies, home-nursing stations, etc. Registration of graphical information, etc. to data centers with that purpose.
  • Response to requests from other medical institutions, etc.
  • In the case of requesting opinions or advice from an external doctor, etc. for the Customer's examination, etc.
  • Explanation of health condition to family, etc.
  • Use in submitting medical bills to a medical bill review organizations, responding to requests from medical bill review organizations or insurers, and other medical bills for healthcare, long-term care, and industrial accident insurances.
  • Use in submitting medical bills to administrative bodies for publicly funded healthcare, responding to requests, and medical billing for other publicly funded healthcare.
  • Notification of results to corporates/companies contracted to the Medical Corporation to perform health examinations, etc.
  • Consultation and submission of forms to specialized organizations and insurance companies related to medical malpractice insurance.
  • Submission of information to external auditing organizations.
  • Research and system development with the purpose of improving healthcare and dissemination of information to the public

4. Utilization for dissemination of information

  • Utilization for dissemination of information such as information from comprehensive health screening center and all the outpatient specialties.

5. Utilization for provision and information of services, and service improvement

Personal information shall be used in services (point system service is also included; the same shall apply hereinafter) provided by the Medical Corporation, Resorttrust, Inc., which is the parent company of the operating company of the Medical Corporation (ie Advanced Medical Care , Inc.) and its subsidiary companies, affiliated companies and alliance medical institutions; for service information; and for survey and research documentation which the aim to improve services provided by the Medical Corporation.

The Medical Corporation will not use the Customer's private information beyond the scope of utilization purposes mentioned above, except in the following cases.
When authorized by the Customer (ie with the Customer's consent)
When an additional utilization purpose is notified or made public
When required by laws and regulations
When permitted by laws and regulations

03 Proper management of personal information

With regards to the Customer's personal information, the Medical Corporation shall strive to keep accurate and up-to-date information, and prevent from the leakage, loss, damage, falsification, and unauthorized access to the Customer's personal information. In the case of entrusting the handling of personal information to an external party within the scope necessary to achieve a utilization purpose, the Clinic shall exercise appropriate supervision over the entrusting party so as to seek the security control of the personal information.

04 Providing personal information to a third party

When the Medical Corporation provide the Customer's personal information to a third party, only limited data categories necessary for each utilization purpose shall be provided by means of document, film, electronic or, magnetic data, or verbal information, etc.; upon obtaining the consent of the Customer, except in cases required by the laws and regulations.

05 Confirmation and disclosure of personal information

If confirmation or disclosure of personal information is requested by the Customer himself or a legally appointed representative, the Medical Corporation shall properly deal with the said request according to the law and the Medical Corporation's policies without delay. For Customers who wish to inquire about the methods and procedures for our handling of such requests, please use the contact information in Section 8.

06 Correction, etc. of personal information

If the customer himself or a legally appointed representative requests for the correction, addition, deletion, utilization cease, removal , or a third-party provision cease of personal information, based on the reason that it is not factual, etc. , the Medical Corporation shall promptly conduct a necessary investigation with regards to the said request and properly handle in accordance with laws and regulations. For Customers who wish to inquire about the methods and procedures for our handling of such requests, please use the contact information in Section 8.

07 Joint utilization

(1) The Medical Corporation, acting as a responsible party for controlling the personal data, will jointly utilize the personal data including medical information among organizations mentioned below (hereinafter referred to as “Jointly Utilizing Corporations”) in accordance with the utilization purposes mentioned in Section 2 ”Utilization purposes of personal data” of this policy.

1. Categories of jointly utilized personal data

Name, date of birth, age, gender, address, E-mail, phone number, clinic ID card number, name of provider medical institution, clinical data (diagnoses, date of consultation, contents of medical record), health screening data (all the images, values, findings, contents of medical interview, medical advice, results)

2. Jointly Utilizing Corporations

Medical Corporation HIMEDIC Clinic WEST; Midtown Clinic Medical Corporation; Trust Clinic Medical Corporation; Keisokai Medical Corporation; National University Corporation the University of Tokyo (The University of Tokyo Hospital); National University Corporation Kyoto University (Kyoto University Hospital); and other medical institutions in alliance with Grand HIMEDIC Club; Shinkokai Medical Corporation
In cases of addition or changes made in the names of the above Jointly Utilizing Corporations, such revisions shall be notified through our web site, in person during the visit to our medical facility or by post.

(2) The Medical Corporation, acting as a responsible party for controlling the personal data, will jointly utilize the following personal data (not including the medical information such as clinic ID number, provider medical institution, clinical data and medical screening data) between the organizations and companies mentioned below (hereinafter referred to as “Jointly Utilizing Organizations”).

1. Utilization Purpose by Jointly Utilizing Organizations

For provision and operation of the point system service, providing information of the said service, and documenting survey or research aiming to improve services.

2. Categories of jointly utilized personal data

Name, date of birth, age, gender, address, E-mail, phone number, membership rights and attributes, purchasing and facility use history, and personal data related to provision and operation of the point system service such as number of points, valid period, use history etc.

3. Jointly Utilizing Organizations

Medical Corporation HIMEDIC Clinic WEST; Midtown Clinic Medical Corporation; Trust Clinic Medical Corporation; Keisokai Medical Corporation; National University Corporation the University of Tokyo (The University of Tokyo Hospital); National University Corporation Kyoto University (Kyoto University Hospital); and other medical institutions in alliance with Grand HIMEDIC Club; Shinkokai Medical Corporation, as well as group companies of Resorttrust Group, which are described in the Annual Securities Report, etc. of Resorttrust, Inc.
