At our clinic, Outpatient care, we will be accepting patients from Tuesday, April 2022, 4.Started supporting "My Number Card Health Insurance Card Use".
Please be sure to check the following before using this service.
●If you have just switched insurance (due to moving, starting a job, retiring, changes in your contribution ratio due to age, etc.), your insurance eligibility information may not be confirmed and you may not be able to use the service.
ContinuedPlease be sure to bring your "old health insurance card" as well.Please.
If you are using publicly funded medical care (medical care for children/medical care for the physically and mentally disabled/medical care for single-parent families, etc.), please continue to bring your paper recipient certificate with you as usual.
*If you only bring your My Number card and we are unable to confirm your insurance eligibility information, you will be responsible for 10% of the cost.We appreciate your understanding in advance.

About the medical information and system infrastructure improvement system enhancement premium
Our clinic calculates a "medical information and system infrastructure development system improvement surcharge" based on the medical fee calculation in the Health Insurance Act.
The "Medical Information and System Infrastructure Development System Enhancement Surcharge" was newly established in October 2022 to evaluate the system in which insured medical institutions provide high-quality medical care by utilizing medical information such as patient medication information and specific health checkup information.
Our clinic has established the following system and strives to provide high-quality medical care by utilizing patients' medical information.

Depending on whether or not you provide medical information via your My Number card at the time of your initial and follow-up visits, the following points will be added to your medical fee assessment.
When you check in, we ask that you cooperate with us by using your My Number card to verify your eligibility online in order to obtain and use accurate patient information.
