Notice of complete transition to online medical examination and health checkups for 2022 (end of paper mailings)

As of April 2022, medical examination and health checkup questionnaires and result sheets will be completely switched over to being entered and viewed online. If you have received them by mail up until now, please note that paper medical examination questionnaires and result sheets will no longer be sent to you. (Excluding some companies and some test items)

If you would like a paper copy of your results, you can print them out in PDF format from the web results page.
*If you would like to have the results sent to you by mail, this is available for a fee. Please contact us by phone or through our customer service desk.
