To patients currently receiving treatment for "high blood pressure," "dyslipidemia," or "diabetes"

Following the revision of medical fees on June 6, 6, we have decided to charge a "lifestyle-related disease management fee" to provide comprehensive treatment management to outpatients whose primary illness is lifestyle-related disease (diabetes, hypertension, and dyslipidemia) instead of the "specific disease treatment management fee" that we have previously charged at our hospital.

For patients who visit the hospital for "high blood pressure," "dyslipidemia," or "diabetes," doctors will create a "lifestyle disease treatment plan" that includes individual goal setting and instruction content, etc., in order to provide better treatment.

When creating this "Lifestyle-Related Disease Treatment Plan," you will be asked to fill out a special questionnaire and also to sign it.
