- Please note that your reservation will not be confirmed by submitting an application through this form.Our staff will call you on the next business day to confirm your reservation. Your reservation will be confirmed once we receive the phone confirmation.
- Reservations can be made from 3 days in advance up to within one month.
- If you do not receive a call by the next business day, we apologize for the inconvenience, but please call us again to confirm.
- If you wish to change or cancel your reservation, please contact us by phone.

For first-time patients
①About reservation time
If you are more than 5 minutes late for your appointment, you may not be able to see us as it will cause inconvenience to other patients. Thank you for your understanding and cooperation.
②Payment
Payment for health insurance treatment is generally accepted in cash.
③ Regarding the preparation of documents brought in by patients, etc.
It will take about one week to prepare the documents and it will cost you money. Please inform our staff as soon as possible. Also, please ask the reception staff about the cost.
④Regarding receipts
We cannot reissue receipts, so please keep them safe at home. If you would like a receipt in English, please let us know in advance. Please note that receipts will only be given in part of Japanese or part of English. Thank you for your understanding.
We also accept reservations by phone
Please contact us by phone to make reservations for treatments that are not covered by online reservations (medical treatments covered by health insurance, orthodontics, implant treatment, etc.).
